Looks like the correct procedure. Of course it it does not work, something is missing. You should try the following:

1.  Open the Asterisk CLI with high verbosity, eg asterisk -rvvvvvvv
2. Try and run the logoin form QM
3. Post here what happened on the CLI

This means that QM was not able to determine that the licence pasted had the format of a valid licence. Very likely, either you left some spaces in, or you forgot a piece of the licence key.

Have you tried including an h target in your dialplan?

This is correct - the Queue field gets populated only if you use AddMember logins, and the Srv field gets popuplated only if you are running a cluster, so calls might be on server A versus server B versus server C....
So I'd say everything is working now :)

Well basically you do not need to "initialize" stats, as you always run a report for a specified period of time.

If you really do and are running SQL storage, here is how:
1. stop qloaderd
2. truncate queue_log table
3. rotate queue_log file in Asterisk
4. restart qloaderd
