The fact that the agent's extension rings means we are at a good starting point :)
Basically what fails here is when it tries to reach an outgoing line.
You should post a CLI trace so we can detect what goes on when it tries to dial outside.

This is the issue:

Problema DB: java.sql.SQLException: Illegal mix of collations (latin1_swedish_ci,IMPLICIT) and (utf8_general_ci,COERCIBLE) for operation '='

Basically either the table or its fields are still with encoding "latin1_swedish_ci" while you should turn it all to UTF-8.
This may apply to the general server settings as well - make sure it is UTF-8.

No, but you can easily find which agents are not following the guidelines to put a status code after each call and address the issue with them.

From what I see, most likely it's not the page having problems but the underlying MySQL instance on tables:

- qa_data
- qa_forms
- qa_forms_items

Please make sure they are all UTF-8, as per http://queuemetrics.com/faq.jsp#faq-036-internationalized-bug231

It may be related to some error on the Asterisk side - do you use the AGI script or old queueDial.agi?

You should use the extension matching - outbound route feature so you can capture all calls with a specific pattern and track that. You should decide a "default" campaign to save the calls to, if you don't want to enter it each and every time.
