QueueMetrics installation / cannot find QA Form and Cfg QA tab

Hi all,

I have asterisk version 1.4.22 with queuemetrics 1.5.0, all were running okay but I cannot enable the QA Form, I can't find Cfg QA on my tab. Please enlightened your humble servant.
